About TRT

What is testosterone replacement therapy?

Testosterone replacement therapy (TRT) is a medically supervised treatment for men with diagnosed low testosterone — a condition known as hypogonadism. It is not a supplement, a steroid cycle, or a lifestyle choice. TRT is a clinical treatment prescribed by a GP after proper blood work and assessment.

How we diagnose low testosterone

Diagnosis requires at least two fasting morning blood tests confirming low total testosterone. Your GP will also order:

  • Free testosterone and SHBG
  • LH, FSH, and prolactin
  • Oestradiol
  • Full blood count (haematocrit)
  • Liver and kidney function
  • Lipid profile and PSA

Treatment options

If blood work confirms clinically low testosterone and symptoms are present, your GP may prescribe:

  • Testosterone undecanoate injections (Reandron) — administered every 10–14 weeks
  • Transdermal gels (Testogel 1%, AndroForte) — applied daily
  • Testosterone creams — compounded formulations applied daily

For men concerned about fertility, alternatives such as enclomiphene — which restores natural testosterone production without suppressing sperm — may be appropriate.

What to expect

Most men notice improvements in energy and mood within 2–4 weeks. Libido typically improves within the first month. Muscle mass and strength take 3–6 months. Full benefits are generally seen by 6–12 months with consistent treatment and monitoring.

Ongoing monitoring

TRT requires regular blood tests — every 8–12 weeks initially, then every 6 months — to monitor testosterone levels, haematocrit, PSA, and liver function. Your GP adjusts your dose based on these results.

Weeks 2–4

Testosterone therapy in Australia is broken. On one side, you've got subscription platforms charging $300–$600 a month for medication you could fill at Chemist Warehouse for a fraction of the price. On the other, most GPs don't have the time or confidence to properly assess and manage TRT.
